Payment is accepted by cash, check, MasterCard, or Visa.  Many of our clients elect to use their HSA (Health Savings Account) or Flex Spending Plan, thereby saving by using pre-tax dollars.  Our facility can only bill Insurance for Medical Massage.  Any discounts made apply to self-referral clinical clients only. Physician-referred medical massage patients are not eligible for discounts.

No physician referral is necessary for the following treatments:

Lymphatic Drainage – Gentle and deeply relaxing treatment developed in Europe.  Excellent for stress reduction, fibromyalgia, chronic fatigue & RDS, detoxification, swelling, lymphedema, sprains & strains, scar reduction & recovery from any sort of surgery.  This is excellent therapy immediately after whiplash as well.

60-75 minute treatment:  $100

120 minute treatment:  $145

Maternity Massage - The mother-to-be requires special and individualized care.  She needs to be comforted and nurtured in a safe way that is mindful of her situation.  Our relaxing and stress reducing maternity massage provides relief from back pain and muscle cramps, reduces swelling and stress on joints as well as anxiety, and increases circulation of blood and lymph systems.

75 minute treatment:  $95

Infant Massage - After the baby comes, our therapist will also be glad to work with the baby, incorporating specific techniques that can encourage healthy development and alleviate discomfort associated with colic.  We especially enjoy helping new parents learn to properly massage their babies.

30 minute treatment:  $45

Structural Integration - The Dr. Ida P. Rolf Method of Structural Integration is a system for balancing the human physical structure by means of fascial connective tissue manipulation and education.  Structural Integration reorganizes the body as a whole by releasing the tensions involved in chronic holding patterns. The work of Structural Integration systematically moves tissue back toward the symmetry and balance that are so clearly required by gravity and the architecture of the body.  While this work has proven to effectively reduce chronic pain, it is fabulous for sedentary, active and athletic people who simply want to feel more present and at ease in their bodies.  Anyone who has a desire to improve movement and posture will benefit.

60-90 minute treatment:  $95

Therapeutic Massage – A more traditional massage customized to the client’s needs – for example, specific pain relief.  Styles of bodywork drawn upon also include CranioSacral Therapy, deep-tissue, hydrotherapy, neuromuscular therapy, Russian sports massage, shiatsu, trigger point therapy, assisted stretching, and Swedish massage.  Generalized full body massage is available, though most clients request that the session focus on a particular area of concern.

60-75 minute treatment:  $95

120 minute treatment:  $140

Head in the Clouds Massage – An extremely relaxing therapeutic massage that focuses exclusively on the head, neck & shoulders.  Complete bliss!  (This is a fantastic option for those who have never received a massage before.)

30 minute treatment:  $55

TMJ Therapy – A gentle and non-invasive treatment alternative for jaw pain.  Contributing postural and lifestyle issues are addressed.  In addition to external work, intra-oral massage is performed.  The course of treatment also incorporates relaxation techniques, specific exercises and stretches.  Each client is educated in useful techniques that can be employed for effective self-treatment.  (NOTE: Our therapist uses latex free gloves.)

60 minute treatment:  $100

Initial Evaluation – Required for all new clients, the comprehensive evaluation allows the therapist to better work with the client to formulate an appropriate treatment plan.  The evaluation includes a detailed health history and postural assessment as well as focused orthopedic testing.

30-60 minute evaluation:  $35

Medical Massage Services available at AMMA

If you have an open liability insurance claim (for a motor vehicle accident or work-related injury, for example) we may be able to bill the insurance company directly.

CPT fees are based upon the 80th percentile of Usual, Customary and Reasonable (UCR) fee schedules for our geographic region. Any discounts made apply to self-referral clinical clients only. Physician-referred medical massage patients are not eligible for discounts.

Medical Massage – Medical massage follows the treatment plan dictated by the diagnosis given and procedures prescribed by the physician.  Procedures employed include manual therapy techniques, neuromuscular re-education, massage therapy and therapeutic exercise. Detailed documentation and progress notes are provided to the referring physician, insurance company and attorney as appropriate. Medical massage is not a general full-body treatment.

Medical massage is a valuable supplement to physical therapy and chiropractic care.  Some conditions that it can treat include: whiplash, carpal tunnel syndrome, temporomandibular joint pain (TMJ pain), tennis elbow, golfer’s elbow, sciatica, tendonitis, bursitis, Bell’s Palsy, frozen shoulder, shin splints, plantar fasciitis, and migraines.

CPT 97010 - Cryotherapy, Moist Heat or Topical Pain Relief Application – may include ice massage, contrast baths, or hot/cold packs

CPT 97110 – Therapeutic Assisted Exercise - develops strength, endurance, range of motion and flexibility

CPT 97530 – Therapeutic Activities - direct (one-on-one) patient contact (use of dynamic activities to improve functional performance)

CPT 97535 -Home Management Training (Bandaging, Exercises, Self-Care) - Self-care/home management training (eg, activities of daily living (ADL) and compensatory training, meal preparation, safety procedures, and instructions in use of assistive technology devices/adaptive equipment) direct one-on-one contact

CPT 97161 - Physical Therapy Evaluation: Low Complexity - A history with no personal factors and/or comorbidities that impact the plan of care.  The evaluation includes an examination of body system(s) using standardized tests and measures addressing 1-2 elements from any of the following: body structures and functions, activity limitations, and/or participation restrictions.  This is a clinical presentation with stable and/or uncomplicated characteristics and requires clinical decision making of low complexity using standardized patient assessment instrument and/or measurable assessment of functional outcome.

20 minute evaluation:  $190

CPT 97162 – Physical Therapy Evaluation: Moderate Complexity - A history of present problem with 1-2 personal factors and/or comorbidities that impact the plan of care.  The evaluation includes an examination of body systems using standardized tests and measures in addressing a total of 3 or more elements from any of the following: body structures and functions, activity limitations, and/or participation restrictions.

This is an evolving clinical presentation with changing characteristics and requires clinical decision making of moderate complexity using standardized patient assessment instrument and/or measurable assessment of functional outcome.

30 minute evaluation:  $190

CPT 97163 – Physical Therapy Evaluation: High Complexity - A history of present problem with 3 or more personal factors and/or comorbidities that impact the plan of care.  The evaluation includes an examination of body systems using standardized tests and measures addressing a total of 4 or more elements from any of the following: body structures and functions, activity limitations, and/or participation restrictions.

This is a clinical presentation with unstable and unpredictable characteristics and
requires clinical decision making of high complexity using standardized patient assessment instrument and/or measurable assessment of functional outcome.

45 minute evaluation:  $190

CPT 97164 - Physical Therapy Re-Evaluation - Re-evaluation of the physical therapy established plan of care, involving an examination including a review of history and use of standardized tests and measures as well as a revised plan of care using a standardized patient assessment instrument and/or measurable assessment of functional outcome

20 minute evaluation:  $129
